Full Time Employee Uniform Order Form

  1. Instructions
    Use this form to place your uniform order. Please be aware that full time employees are limited to the following number of items: 2 shirts (may be any combination of type of shirt but total number of shirts may not exceed 2), 2 t-shirts, 1 belt, 1 Job shirt. Any order that exceeds the approved number will be reduced by administration to remain within budget. Please plan your order accordingly.
  2. Certification
    Please indicate your certification level as you wish it to appear on your uniforms.
  3. T-Shirt Ordering Instructions

    Please be aware that you are limited to a maximum of 3 t-shirts. 

  4. T-Shirt

    Select the appropriate size t-shirt. Please ensure that the size you order is of sufficient size to remained tucked in at all times.

  5. Please indicate the number of items needed. Only order what is necessary.
  6. Shirt Ordering Instructions

    When ordering polo shirts or button-down shirts, if the button-down is your primary uniform shirt you may order 3 and are limited to 2 polos. If the polo is your primary uniform shirt you may order three and are limited to 2 button-down shirts. You may only have 1 button-down shirt with subdued patches.

  7. Polo

    Select the appropriate size polo. Please ensure that the size you order is of sufficient size to remained tucked in at all times.

  8. Gender
    Please indicate the gender cut of the item you wish to order.
  9. Please indicate the number of items needed. Only order what is necessary.
  10. Shirt Ordering Instructions

    When ordering polo shirts or button-down shirts, if the button-down is your primary uniform shirt you may order 3 and are limited to 2 polos. If the polo is your primary uniform shirt you may order three and are limited to 2 button-down shirts. You may only have 1 button-down shirt with subdued patches.

  11. Button Down Shirt

    Select the appropriate size button-down shirt. Please ensure that the size you order is of sufficient size to remained tucked in at all times.

  12. Please indicate the number of shirts with subdued patches needed (limit 1). Only order what is necessary.

  13. Please indicate the number of shirts with full color state patches (limit 2). Only order what is necessary.

  14. Job Shirt

    Please indicate the size of job shirt you wish to order. Please be aware that the 5.11 brand job shirt tends to run long while the Propper tends to run much shorter. Please keep this in mind when determining the brand you wish to order. The 5.11 job shirt is unavailable in sizes 4X-Large and higher.

  17. Pant Ordering Instructions

    Please be aware you are limited to a maximum order of 3 pants.

  18. Please indicate the waist size and hem length (in that order) of the pants you wish to order.
  19. Pant Brand
    Please indicate the brand of pant you wish to order.
  20. Gender Cut
    Please indicate the gender cut of pant you wish to order.
  21. Please indicate the number of items needed. Only order what is necessary.
  22. Lounge Shorts

    Select the appropriate waist size of shorts you wish to order.

  23. Please indicate the number of lounge shorts you wish to order.

  24. Please indicate the size of belt you wish to order (limit 1).

  25. You may only order boots with prior approval from your supervisor. Limit 1 pair per year. Any boot order placed without supervisor approval will be cancelled without notification to the employee. If approved to purchase boots, please indicate the brand, model number, and size in this field. Brands limited to: Bates, Haix, Blauer, 5.11, Thorogood, and Rocky.

  26. You may only order a jacket with prior approval from your supervisor. Any jacket order placed without supervisor approval will be cancelled without notification to the employee. If approved to purchase a jacket, please indicate the size needed in this field.
  27. Please indicate the number of items needed. Only order a new safety vest if yours is damaged and needing replacement.

  28. Please indicate the number of Velcro name patches needed for your jacket and safety vest. Limit 4.
